Documents & Information to send to Curran & Berger for your PERM petition.

This checklist does not address the question of whether it is advisable to file a PERM petition but is intended for current clients who have been referred to this list by Curran & Berger staff.

This is a comprehensive list of everything Curran & Berger will need to begin work.  We realize you may have sent us some of this information already.  In that case, you do not need to send it again. If certain items are difficult or impossible to get, send us as much as you have.

Please provide our office with the following documentation.  All documentation not in English must be accompanied by a Certified English translation.  Please contact our office if you need a translation certification form.

1. Current detailed resume or CV
2. Completed education and employment history form 
3. Copies of all diplomas, degrees, transcripts, and certificates
4. Education evaluation (if applicable and available)
5. Complete copy of passport (including front and back cover, all blank pages and I-94 card)
6. Complete copy of detailed immigration history and related documents, including non-immigrant documents, student documents, employment documents, etc.
7. Copy of W-2 and pay-stub from current employer
8. Marriage certificate with certified translation, if applicable
9. Children’s birth certificates with certified translation, if applicable
10. Principal Applicant’s birth certificates with certified translation, if applicable
11. Spouse’s birth certificates with certified translation, if applicable

1. Address in the US and your address in your home country
2. Date of birth
3. City and country of birth
4. Current Nationality or Citizenship
5. Phone number (work and home)
6. What is your current immigration status?
7. When does your current immigration status expire?
8. Have you ever been through deportation proceedings? If yes, explain.
9. Have you ever been arrested and/or convicted of anything here or abroad?
10. Do you have a spouse and/or children?  If yes, please provide: names, date of birth, place of birth, citizenship, and copies of all immigration documents.
