Documents & Information to send to Curran & Berger for your H-1B petition.

This checklist does not address the question of whether it is advisable to file an H-1B petition but is intended for current clients who have been referred to this list by Curran & Berger staff.

This is a comprehensive list of everything Curran & Berger will need to begin work.  We realize you may have sent us some of this information already.  In that case, you do not need to send it again. If certain items are difficult or impossible to get, send us as much as you have.

Please provide our office with the following documentation.  All documentation not in English must be accompanied by a Certified English translation.  Please contact our office if you need a translation certification form.

DOCUMENTS:

  1. Current detailed resume or CV
  2. Copies of all diplomas, degrees, transcripts, and certificates (with Certified English Translation if applicable)
  3. Complete copy of passport (including front and back cover, all blank pages and I-94 card)
  4. Complete copy of detailed immigration history and related documents, including previous H-1B visas, IAP-66, I-20, student documents, employment documents, EAD, etc.
  5. Copy of pay-stub from current employer
  6. Copy of ECFMG certificate
  7. Copy of valid medical license (from the state you intend to work as an H-1B employee)
  8. Copy of US-MLE parts 1, 2, and 3 score reports


*If you intend to apply for permanent residence status, you may want to obtain a copy of your birth certificate if it is not already here in the U.S.

INFORMATION:

  1. What is the US Consulate nearest to you in your home country? (make sure they process H-1B’s)
  2. Social Security number
  3. Address in the US and your address in your home country
  4. Phone number (work and home)
  5. Fax number
  6. Email
  7. Does your name appear the same on all of your documents such as your passport, birth certificate, and marriage certificate?  If not, please list the different variations of your names.
  8. Do you have a spouse and/or children; will they be applying with you?  If yes, see H-4 section below and complete the information requested.
  9. What is the date and place of your last entry into the US? (Can also be found on your most recent I-94 card)
  10. What is your current nonimmigrant status?
  11. When does your current visa expire?
  12. Were you inspected by an INS officer during your last entry into the US?
  13. Have you ever been through deportation proceedings? If yes, explain.
  14. Have you ever been arrested and/or convicted of anything here or abroad?
  15. Do you and/or your dependents have any plans to travel outside the U.S. within the next 6 months?


H-4 Checklist (documents and information needed for your spouse and children who will be applying for H-4 visas)

DOCUMENTS:

  1. Complete copy of passport (including front and back cover, all blank pages and I-94 card)
  2. Complete copy of detailed immigration history and related documents, including previous H-1B visas, IAP-66, I-20, EAD, etc.
  3. Marriage certificate with certified translation, if applicable
  4. Children’s birth certificates with certified translation, if applicable

*If your spouse intends to apply for permanent residence status, he/she may want to obtain a copy of his/her birth certificate if it is not already here in the U.S.

Information:

  1. Your spouse and child’s social security numbers (if any)
  2. Their addresses, if different from yours
  3. The country of their citizenship
  4. The dates and places of each of their last entries into the US
  5. Do their names appear the same on all of their documents such as their passport, birth certificate, and marriage certificate?  If not, please list the different variations of their names.
